Rectal cancer management - What are the stages of colon cancer - How to treat rectal cancer

Rectal cancer management requires a multimodality approach and this talk by Dr. Gunjan Desai of liver and digestive diseases clinic, Mumbai and Navi Mumbai outlines the approach beautifully in the fourth webinar of Edusurg Clinics on non-metastatic rectal cancer management.

The talk begins with the anatomy of the rectum which includes the sub-divisions of the low rectum, mid rectum, and upper rectum, lymphatic drainage of the rectum as well as the concept of mesorectum and total mesorectal excision. The video then highlights the clinical presentation including the rectal cancer symptoms as well as the signs followed by the workup and tests that are required to establish the diagnosis as well as stage the disease.

The talk then focuses on the aims of treatment which predominantly include sphincter preservation and oncological safety. He gives a rectal cancer treatment protocol and summarizes the updates in rectal cancer management as well. The role of long course and short course neoadjuvant radiation, as well as chemotherapy, are discussed and selection criteria elaborated. This management outline will definitely answer the question of which neoadjuvant treatment in rectal cancer to give and when.

The talk then focuses on surgical treatment options for rectal cancer based on Rullier's classification of low rectal tumors and describes the principles of Bill Heald's Total mesorectal excision surgery. It also describes the three critical sites of injury during TME and advocates Nerve-sparing TME. New techniques for rectal cancer surgery are also touched upon towards the end. The talk describes the surgical management of rectal cancer in a very simplified, yet descriptive manner.
